Hospice care referrals across North Hampshire

St. Michael’s Hospice accepts referrals from patients across North Hampshire and, in certain exceptional circumstances, from outside this catchment area. As the only hospice serving North Hampshire, St. Michael’s provides specialist palliative and end-of-life care to a population of approximately 300,000 people.

Our hospice supports patients, families, and carers by providing compassionate care, medical support, and wellbeing services tailored to individual needs.

Useful information about our catchment area

Eligibility for referral to St. Michael’s Hospice is determined by the catchment area of your GP surgery, rather than your home address. If your GP practice is located within our service area, you may be referred to our hospice services.

Patients registered with the following North Hampshire GP surgeries are within the St. Michael’s Hospice referral area:

  • Alresford Surgery
  • Boundaries Surgery
  • Bramblys Grange Medical Practice
  • Camrose, Gillies, Hackwood Partnership (including Beggarwood Surgery)
  • Chawton Park Surgery
  • Chineham Medical Practice
  • Clift Surgery
  • Crown Heights Medical Centre
  • Holmwood Health Centre (including Tadley and Morlands surgeries)
  • Mansfield Park Surgery
  • Shakespeare Road Medical Practice (including Bermuda Surgery and Marlowe Surgery)
  • The Odiham and Old Basing Health Centres
