Make a referral to St. Michael’s Hospice

St. Michael’s Hospice provides specialist palliative and end-of-life care in North Hampshire, supporting patients with life-limiting conditions and their families.

Healthcare professionals can refer patients quickly and easily to access a range of our services, ensuring they receive the right care at the right time. Referrals can be made by any healthcare professional using a simple process, helping our team assess patient needs and provide compassionate, personalised support.

Making a referral

The easiest way to be referred is for a healthcare professional to download and complete the below referral form. Once completed, the healthcare professional should email the form to: St. Michael’s Hospice Palliative Care Office: smhadmin@hhft.nhs.uk or Basingstoke and North Hampshire Hospitals Community Palliative Care Team: palliativecare@hhft.nhs.uk

Your healthcare professional will need to provide as much information as possible on the form to enable the team to make informed decisions and an effective assessment.

Healthcare professionals can also make a referral by letter, email, face-to-face or by telephone and the information will be transferred to the form.
